Once accepted into the Clinical Lactation Training program, BreastfeedLA requires the following documents to be submitted prior to placement.
✅ Health screening/clearance
✅ Criminal background check
✅ OIG Sanction Trace
✅ Proof of malpractice insurance coverage
✅ Seasonal flu shot/declaration of religious exemption
✅ Proof of TB screening
✅ Immunization Records (including MMR, varicella, Hep B, TDaP)
✅ Pathway pre-approval from IBLCE
✅ Completion of HIPAA training, Universal Precautions & Infection Control, Medical Documentation, Medical Terminology, Professional Ethics, Basic Life Support (BLS)
*All required documentation, health screenings, and approvals must be submitted prior to placement. In the event that program requirements are not met prior to program start date, student will be removed from the program and all fees paid to BreastfeedLA to date will be forfeited.
